Ian L., Dr -

How valuable then is examining a child of 60 days normal term as to ability to support head and upper body when on its stomach or following moving objects with its eyes in determining illness severity?
Could we teach PARENTS and caregivers givers the core elements of Triage Tools like the Pediatric Assessment Triangle to strengthen the safety of childcare?

Tracy G. -

From Ilene Claudius: I think it is very valuable to look at things like tone and attentiveness as you suggest when evaluating the febrile child. Unfortunately, only about half of kids <90d with a fever look ill. Taking, for example, SBIs in kids 1-2 mo, the risk is 1.2-2% if well-appearing and 10-11% if ill-appearing. So looking ill (poor tone, poor attentiveness) means alot. But being well-appearing alone doesn't exclude SBI. Regarding parent education, the PAT (though poorly studied) makes alot of sense and is very user-friendly. I suspect it would be helpful in many settings. At my shop, we see multiple patients for months of unchanged discomfort and fevers to 99, so we are far from tackling that one!
