SD - I wouldn't worry much about using ketamine. The old thought on ketamine was about it causing a catecholamine surge. It can lead to release of endogenous catecholamines but, these patients are going to already be maximally stimulated and it's unlikely they've got any catecholes left. Ketamine is a good choice here particularly if you're trying to do an approach for intubation with no paralytic.

what's the current data on Ketamine in those with CAD? I assume most people with critical aortic stenosis would have CAD.
