Old School Advice. The tongue hanging out is a very challenging intubation, which can go south fast. You can "protect" the airway by grabbing the top & bottom of the tongue with a 4X4 and pulling. This can be sustained for quite a while while waiting for anesthesia Patient can breathe. As bad as the swollen tongue looks, there is not generally respiratory distress. No reason for us to create that.
