I just had this conversation the other day with Ob. We have the " heart beat bill in Ga." Folks get a little jumpy when we start talking about MTX these days.
Quant of 2000, RLQ pain, hemodynamically stable in a patient desiring pregnancy. OB wanted to wait and repeat the HCG. Uptodate, seems to agree. The recommendation is to wait until the quant is ~3500 (again, in the hemodynamically stable patient).
In a hemodynamically stable patient who looks well and has rapid access for follow up, this is a completely reasonable approach when the uterus is empty and the patient desires the pregnancy. I would add to that that the US shouldn’t show free fluid and the patient’s pain should be mild. -Swami
