If you suspect a TM rupture, suspension is recommended. Solution can cause more burning and stinging. However, if you don't have suspension, it's not the end of the world and you can use solution in a pinch.
I tried doing some digging online but couldn't come up with an answer--is there a reason why you wouldn't just use suspension on everyone when available? Is solution more effective, lower cost, more readily available, etc?
I should have looked at CorePendium before commenting! Looks like the solution has acetic acid which helps reduce pH and facilitates elimination of bacteria, however this is also what causes the increased irritation/pain

Typically in these patients the ear canal is so swollen that you cannot visualize the TM. In this case would you recommend using the suspension?

In Canada we use Ciprodex which is ciprofloxacin and dexamethasone.
