Here is a slide from my ACEP Pedi Airway talk.

These are recommendations but I usually start a bit lower (3/4 what is recommended) then titrate up quickly. Key is not what you start at but titration up or down based on how the kid looks. Usually play with flow while watching respiratory rate and retractions.
