What is the situation with adjusted CPAP WITH CPAP mask viral filter to PEEP AMBU BAG and TWO oxygen inputs -the ambubag and the stem connecting CPAP with AMBU-BAG as shown in video Em -CRIT - Scott Weingart . -Nebulisation in the open air ought be less dangerous -IM epinephrine and subcut terbutaline have been advocated for asthmatics but would it be ok for exacerbation copd

Thank you for your questions! In order... 1) The dual CPAP/Ambu bag system you mentioned, although appearing effective, may be considered too complex to employ in most EMS systems, particularly given the current limitations in hosting in-person trainings to ensure quality control. 2) Nebulization outside may dissipate faster, however, many EMS calls take place in small, enclosed spaces, not to mention the transport time in an enclosed ambulance, thus still creating exposure risk. But, like with many of the items discussed in the episode, everything is a factor of risk vs. benefit. 3) Unlike Epi which is widely available in most EMS systems, terbutaline is less so. The EMS are not being protected -the minimum is a mask face visor gloves and gowns and you don't see this on the broadcasts . I have seen up to five EMS needed to stretcher or chair patients down stairs or into lifts on the broadcasts -it is unbelievable . -
