I had this near identical case. The patient went for PCI.

super excellent discussion. remarkable , and unfortunate, cases. its even perhaps more challenging in my shop (merced, California) where the strokes with LVO's get choppered to one place (Modesto), while the STEMI's usually go to a closer PCI hospital in Turlock. no one ever said it was going to be easy, I guess.
