I just listened to the C. Diff talk by Dr. Long and it was great. One point not mentioned has to do with the testing and even more importantly, not testing pediatric patients. Asymptomatic colonization with toxigenic C. difficile is common in neonates and infants. The IDSA has a nice blurb about which pediatric patients need testing. Keep up the excellent work!
It is always educational to speak about the work up of even garden variety cases but there is a rare case of leg pain that should be mentioned Although it is a form of a DVT - phlegmasia cerula dolens deserves mention because it does require acute intervention
I listened to the Pediatric Pearls: Unvaccinated Kids. Unfortunately the segment left me without specific answers about what to do when you work in areas with limited vaccination rates (85 to 88% in the counties that I work in Western Washington states). I had previously come from Michigan where vaccination rate approached 100%. But here I have seen toddlers that looked great initially that came back positive for HIB or Strep Pneumonia. Having more specific protocols for under vaccinated areas will help. Also the segment also talked about under 2 year old. What about over 2 years that are unvaccinated without herd immunity.
