I’d love to see the studies about synovial lactate. 1st time I hear about that!

Greg J. -

EMRap talks such as the one about influenza testing make me a little nuts. No critical analysis of the CDC recommendations for antivirals, just recitation of what CDC says? Is there solid literature supporting Tamiflu use in those "at risk" patients they identify? If so, I'd sure like to hear about it, because I've never seen it! Don't think it's unreasonable for EMRap subscribers to expect actual analysis of the literature basis for these types of recommendations.

Stuart S., MD FAAEM -

Greg - we agree. In addition to this months Annals piece there is also an influenza pathway treatment discussion with Cam Berg in EMRAP February 2017. Neither adequately answers your questions about the quality of the evidence. We plan to revisit flu in the fall at the beginning of next year's season and we will put your specific questions directly to our expert, Dr. Greg Moran. Also, keep an eye on the EMA database - any papers of relevance will also show up there. There is a discussion in EMA June 2015, "Oseltamivir Treatment For Influenza In Adults: A Meta-analysis Of Randomised Controlled Trials" with Jerry Hoffman and Ken Milne that casts serious doubt on its efficacy. Thank you again for your message!
