An internuclear ophthalmoplegia is a disconjugate lateral gaze in which the affected eye cannot adduct. You can see this patient's right eye is affected, so he is unable to adduct his right eye when looking to the left. In other words, his right eye cannot look to the left. There are many possible etiologies with the most common causes being infarction and multiple sclerosis.
