The other drug that is useful to use when treating AF with underlying WPW is flecainide. Obviously DC cardioversion may be the way to go with hypotension etc but I have seen one patient who failed to revert following 2 x DC shocks but subsequently reverted after 50mg IV flecainide (as a slow push). max dose seems to be about 1-1.5mg/kg.
