Ectopic Pregnancy

  • Presents around 5-8 weeks
  • Risk factors
    • Previous ectopic
    • Hx of PID
    • IUD
    • Tubal ligation
    • Diethylstilbestrol
  • Symptoms
    • Unilateral adnexal tenderness +/- bleeding
    • Referred shoulder pain (blood in peritoneum)
  • Investigations
    • bHCG
    • U/S
    • Considered an ectopic if
      • bHCG > 6000-6500 + abdominal U/S with empty uterus
      • bHCG > 1200-1500 + vaginal U/S with empty uterus
    • If bHCG < 1200 → repeat bHCG in 48 hours.
  • Treatment
    • Unstable patients → OR
    • If pregnancy < 4 cm and patient stable → consider methotrexate in consultation with ob-gyn
  • Consider heterotopic pregnancy (IUP + ectopic) in IVF patients
