Once you’ve decided to administer the rabies immune globulin, calculate the dose, which is 20 IU/kg, and make sure the wound has been thoroughly cleaned. Inject directly at the site of the wound, making sure your needle tip is in tissue, not in the laceration, so the antibodies infuse into the tissue. Withdraw and redirect your needle to fan out the distribution of the antibodies. Inject into each site of the bite wounds if there’s more than one. If there is any remaining volume it can be given IM at a different site than the rabies vaccine. Here’s a case of a high risk wound in a fingertip. Only about half an mL was injected in the finger and the rest had to go IM.
